205 / AQ-H02 — Coating Barrier Reinstatement & Return-to-Service Hold
WHEN AND WHY TO USE IT
This is a controlled hold/release record, not permission to restart an operating process. Removing insulation or fireproofing can temporarily defeat a heat, weather, corrosion or process-protection barrier. The physical coating may look complete even though the exposed substrate, applied thickness or curing has not been accepted for return to service.
THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS
The six printed steps have an EVIDENCE / ITP REF. and RELEASE BY / DATE for each HOLD. 1. Affected facility barrier, process boundary or protection system identified and controlled.; 2. Check hazardous-area classification, coating SDS, containment and CUI inspection access; prevent drift into process air intakes.; 3. Approved technical / engineering inspection, tests and quality evidence obtained.; 4. Hold on substrate condition, coating thickness/cure and fireproofing reinstatement before replacing insulation or access covers.; 5. Outstanding open action, retest or failed inspection retained as HOLD / NO RESTART.; 6. Workface verified clean and accounted for; operations authorises reinstatement / return.
HOW TO USE IT PROPERLY
Identify the actual plant/equipment, work order/PTW, contractor/shift; each of six rows requires EVIDENCE/ITP REF. and RELEASE BY/DATE. Record coating/substrate photographs, thickness test, fireproofing acceptance, OPEN DEFECT/NCR/HOLD and REINSPECTION & FORMAL RELEASE refs, plus separate contractor and facility authority signatures/time. Enter open DEFECT/NCR/HOLD and REINSPECTION & FORMAL RELEASE references, and obtain contractor and facility-authority signatures and date/time.
WHAT HOLDS THE WORK
Do not close over an unresolved CUI defect, failed coating cure or unaccepted fireproofing. Operator-controlled isolation, permit clearance and process restoration cannot be substituted by a signed contractor hold sheet.
