204 / AQ-H01 — Surface Preparation & Coating Inspection Release Holds

WHEN AND WHY TO USE IT

This is a controlled hold/release record, not permission to restart an operating process. A refinery coating system depends on approved substrate preparation, compatible materials, witnessed tests and correct fireproofing and insulation acceptance. A hold point prevents the next layer from concealing a defective surface or unaccepted critical process barrier.

THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS

The six printed steps have an EVIDENCE / ITP REF. and RELEASE BY / DATE for each HOLD. 1. Surface preparation scope/permit; 2. Chemical/coating/material approval; 3. Cleaned surface inspection; 4. Coating thickness/cure witness; 5. Fireproofing and insulation acceptance; 6. Waste and plant return

HOW TO USE IT PROPERLY

For each of six work stops, record EVIDENCE / ITP REF. and the authorised RELEASE BY / DATE. File site-approved SDS/material details, surface-preparation records, cure/DFT witness evidence and closing waste/control records. Use the original OPEN DEFECT/NCR/HOLD and REINSPECTION & FORMAL RELEASE lines, with contractor/facility-authority signatures. Enter open DEFECT/NCR/HOLD and REINSPECTION & FORMAL RELEASE references, and obtain contractor and facility-authority signatures and date/time.

WHAT HOLDS THE WORK

A failed inspection or missing material release cannot be ignored. Plant protection, permits and final return remain subject to the facility operator and relevant competent quality authority.
