203 / AQ-C03 — Coating Unexpected Condition STOP-WORK Checklist

WHEN AND WHY TO USE IT

This is an eight-point yes/no/not-applicable inspection and decision tool, not a self-issued site permit. Apply this process-deviation checklist if solvent vapours, abrasive dust, corrosion-under-insulation findings or work near hazardous equipment changes the accepted method, process boundary, ignition controls or inspection decisions.

THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS

The printed table provides eight numbered PROCESS / SAFETY VERIFICATION rows with Y, N and N/A decisions. 1. Current plant status and workface boundary reconfirmed with the facility area owner before activity.; 2. Solvent ignition, abrasive dust or corrosion-under-insulation exposure.; 3. Check hazardous-area classification, coating SDS, containment and CUI inspection access; prevent drift into process air intakes.; 4. New process, energy, chemical, ignition or SIMOPS condition recorded and work stopped pending reassessment.; 5. Operator-controlled permit and isolation validity reconfirmed; no informal supervisor-only restart.; 6. Affected adjacent trades, area operations and emergency response personnel advised of the deviation.; 7. Engineering / environmental / quality nonconformity disposition filed where the change affects integrity.; 8. Competent contractor supervisor and operator area authority jointly document permitted restart or cancellation.

HOW TO USE IT PROPERLY

Identify facility/unit/equipment, operator PTW, work order, contractor and shift. Mark each eight-row Y/N/N/A result, explain genuine N/A, record evidence, open actions and permit variation. Contractor and facility authority separately sign/date the result. The original form includes evidence/open-action/permit variation and separate contractor/facility authority names, signatures and date/time. Justify any N/A rather than merely ticking boxes.

WHAT HOLDS THE WORK

An unapproved coating change, undisposed damage or uncertain hazardous-area condition requires STOP/HOLD; an informal supervisor restart does not remove the hold.

WHAT YOU RECEIVE AND OPTIONS
