202 / AQ-C01 — Hazardous-Area Coating & Insulation Prestart Checklist

WHEN AND WHY TO USE IT

This is an eight-point yes/no/not-applicable inspection and decision tool, not a self-issued site permit. Before abrasion, paint mixing, solvent application, fireproofing removal or insulation access in an operating refinery, the workface needs verification of chemical exposure, ignition zones, affected process assets and quality inspection releases.

THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS

The printed table provides eight numbered PROCESS / SAFETY VERIFICATION rows with Y, N and N/A decisions. 1. Material/paint SDS, flash point and occupational exposure assessed; 2. Surface preparation can affect process integrity and contamination; 3. Abrasive dust/silica and lead/asbestos hazards screened; 4. Ignition/solvent vapour control and classified areas verified; 5. Fireproofing/coating removal requires acceptance before exposure; 6. Specified surface prep, coating and curing records ready; 7. Insulation and corrosion-under-insulation inspection hold addressed; 8. Waste, drift and final coating test accepted

HOW TO USE IT PROPERLY

Use actual equipment tag, site work order/PTW, approved material and SDS, permit boundary, expected environmental containment and Q/C test plan. Complete source eight Y/N/N/A boxes with evidence/open actions/permit variations; separate contractor/operator signatures and date/time are printed. The original form includes evidence/open-action/permit variation and separate contractor/facility authority names, signatures and date/time. Justify any N/A rather than merely ticking boxes.

WHAT HOLDS THE WORK

Do not commence if the asbestos/lead screening, compatible system, dust/ignition controls or plant permit is absent. Hold and escalate to the area authority before work.
