201 / AQ-C00 — Coatings, Insulation & Fireproofing Activation Gate

WHEN AND WHY TO USE IT

This is an eight-point yes/no/not-applicable inspection and decision tool, not a self-issued site permit. Use this reviewer-facing mobilisation gate before accepting a coating, blasting, fireproofing or insulation contractor at a live refinery. It establishes who owns the service, where it can be performed, and whether hazardous materials, access restrictions, inspection and plant-issued permit types have been agreed.

THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS

The printed table provides eight numbered PROCESS / SAFETY VERIFICATION rows with Y, N and N/A decisions. 1. Facility service owner and work order confirm this specific trade service; 2. Material/paint SDS, flash point and occupational exposure assessed; 3. Surface preparation can affect process integrity and contamination; 4. Abrasive dust/silica and lead/asbestos hazards screened; 5. Ignition/solvent vapour control and classified areas verified; 6. Fireproofing/coating removal requires acceptance before exposure; 7. Relevant drawings, inspections and permit types agreed with site issuer; 8. Supervisor and operator acceptance recorded before mobilisation

HOW TO USE IT PROPERLY

Record facility/unit/equipment, operator work order and PTW, contractor/shift, actual source evidence for eight Y/N/N/A gates and any open action/permit variation. Both designated contractor supervisor and facility-authority sign/date before mobilisation. The original form includes evidence/open-action/permit variation and separate contractor/facility authority names, signatures and date/time. Justify any N/A rather than merely ticking boxes.

WHAT HOLDS THE WORK

An unapproved material, unidentified process protection barrier, inappropriate ignition control or unavailable permit leaves the mobilisation at STOP/HOLD. The original gate is not a plant-issued permission.
