192 - AP-C01 | HVAC & Mechanical Services Workface Prestart Checklist
WHERE / WHEN & WHY THIS DOCUMENT IS USED
Use this at the actual refinery workface immediately before the task starts, after site-specific instructions and permits have been identified. It checks the method, equipment, process interface and trade-specific controls against current plant conditions—not a generic site induction. Here the trade is hvac and mechanical services, with a source-identified process exposure of ventilation shutdown accumulates toxic or flammable gas. Link the checks to actual equipment tags and the current facility-authorised scope.
THE SOURCE-PRINTED CONTROLS
The document contains EIGHT printed PROCESS / SAFETY VERIFICATION rows, each with Y, N and N/A decision boxes: (1) Facility room/zone classified for flammable/toxic process atmospheres; (2) HVAC ventilation shutdown impact to process and people assessed; (3) Refrigerant type, leak risk and recovery plan accepted; (4) Duct/fan isolation and rotating equipment controls in place; (5) Ventilation changes cannot degrade confined-space or classified-area safety; (6) Pressure testing, brazing/hot work permit screening completed; (7) Condensate/drain handling and bacterial control assessed; (8) Functional test and ventilation reinstatement accepted.
HOW TO COMPLETE THE RECORD
Enter FACILITY / UNIT / EQUIPMENT, the true work order and operator PTW reference, plus contractor/shift. Inspect actual conditions, record EVIDENCE / OPEN ACTION / PERMIT VARIATION, and explain any N/A. The form requires separate CONTRACTOR / NAME & SIGN and FACILITY / AUTHORITY & SIGN with DATE / TIME. Keep underlying inspection, SDS, test or permission evidence as relevant to these exact controls.
STOP, HOLD & AUTHORITY LIMITS
The original HOLD RULE is explicit: an unanswered critical item, absent plant permit or unapproved deviation requires STOP/HOLD. A contractor tick, internal signature or completed checklist does not authorise release from an operating refinery. Resolve deficiencies with the competent technical reviewer and facility operator, and record the approved decision before progressing.
TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE
Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
