184 / AK-R02 — MINE FIRE PROTECTION DEFECT AND CORRECTIVE ACTION LOG
AK-R02 is the original editable five-column DEFECT, INSPECTION AND CORRECTIVE ACTION LOG for mine fire detection, suppression and life-safety systems. Its purpose is to document which protection is still missing, who must restore it and whether the mine fire authority has accepted closure.
THE REAL MINE FIRE AND LIFE-SAFETY DECISION
A failed fire pump, bypassed detector or incomplete alarm test may persist while occupied areas and refuelling operations continue. Without a documented correction owner and mine-accepted closeout, an issue can disappear from the shift handover before effective fire protection is restored.
FIVE ORIGINAL DEFECT AND ACTION FIELDS
1. ITEM / LOCATION identifies affected alarm, sprinkler, pump or suppression asset. 2. DEFECT / CONTROL records the failed fire or life safety safeguard. 3. OWNER / TARGET assigns competent repair accountability.
4. ACTION / EVIDENCE captures repair, test, photograph or service certificate. 5. ACCEPTED / CLOSED records authorised reinstatement or restriction.
EDITABLE SOURCE WORD RECORD AND AUTHORITY
The actual printed FIVE columns are ITEM / LOCATION, DEFECT / CONTROL, OWNER / TARGET, ACTION / EVIDENCE and ACCEPTED / CLOSED. Identifiers include Mine/site, Company, Contract/work order and Responsible person. Retain photographs, inspection records and technical certificates. This is not a seven-point test sheet or authority to cancel an impairment notice.
Maintain actual source evidence, mine permit, alarm panel test reports, valve tags and defect/reinspection closeout. An unverified bypass cannot be accepted as a restored fire system.
ONE EDITABLE SINGLE OR FULL FIRE SAFETY FILE CONTENT?
This same source-identical purchaser-licensed editable Word DOCX is ALREADY INCLUDED in the 386-document Surface Mining Checklist Package and in the complete Surface Mining Contractor Health & Safety File. Purchasers of either comprehensive collection already receive this fire-system original alongside other specialist forms and mine safety controls; they need not buy the Single separately. Mine fire authority controls any impairment and reinstatement.
