182 - AO-C01 | Scaffolding & Work at Height Workface Prestart Checklist

WHERE / WHEN & WHY THIS DOCUMENT IS USED

Use this at the actual refinery workface immediately before the task starts, after site-specific instructions and permits have been identified. It checks the method, equipment, process interface and trade-specific controls against current plant conditions—not a generic site induction. Here the trade is scaffolding and work at height, with a source-identified process exposure of scaffold or rope system obstructs process access or damages piping. Link the checks to actual equipment tags and the current facility-authorised scope.

THE SOURCE-PRINTED CONTROLS

The document contains EIGHT printed PROCESS / SAFETY VERIFICATION rows, each with Y, N and N/A decision boxes: (1) Height access and competent designer/inspector duties screened; (2) Scaffold load, plant obstructions and insulation/pipework clearance verified; (3) Live vents, steam and chemical discharges considered for elevated work; (4) Anchor/fall arrest/rescue methods accepted; (5) Scaffold tags, inspection and weather status current; (6) Dropped object nets/exclusion below work area established; (7) Emergency access and fire service routes maintained; (8) Temporary structures removed and area inspected.

HOW TO COMPLETE THE RECORD

Enter FACILITY / UNIT / EQUIPMENT, the true work order and operator PTW reference, plus contractor/shift. Inspect actual conditions, record EVIDENCE / OPEN ACTION / PERMIT VARIATION, and explain any N/A. The form requires separate CONTRACTOR / NAME & SIGN and FACILITY / AUTHORITY & SIGN with DATE / TIME. Keep underlying inspection, SDS, test or permission evidence as relevant to these exact controls.

STOP, HOLD & AUTHORITY LIMITS

The original HOLD RULE is explicit: an unanswered critical item, absent plant permit or unapproved deviation requires STOP/HOLD. A contractor tick, internal signature or completed checklist does not authorise release from an operating refinery. Resolve deficiencies with the competent technical reviewer and facility operator, and record the approved decision before progressing.

TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE

Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
