AK-H02 is the original editable FIVE-GATE MINE TECHNICAL WITNESS AND RESTRICTED RELEASE form for work on fire detection, suppression and life-safety equipment. It places the mine fire owner's approved impairment and compensating controls at the first gate.
THE REAL MINE FIRE AND LIFE-SAFETY DECISION
When protection is bypassed for maintenance, continuing hot work or fuel transfer without a fire watch can increase risk across multiple mine zones. Even a new detector or valve can be inappropriate for the installed alarm/pressure system. Independent fire authority acceptance is necessary before release.
FIVE WITNESSED TECHNICAL HOLD GATES
1. Mine fire owner approves impairment zone, scope and compensatory fire watch. 2. Identify fire water, alarm, fixed-agent and mobile plant suppression dependencies. 3. Controlled tag and expiry for every bypass, isolation or disabled function.
4. Review hot work or fuel delivery hazards during reduced protection. 5. Certified replacement components and pressure/detector suitability verified.
