181 / AK-F01 — MINE FIRE SYSTEM FIVE-STAGE HOLD, RELEASE AND HANDBACK

AK-F01 is the original editable five-stage HOLD / RELEASE AND HANDBACK form for maintenance on critical surface-mine fire systems. It requires a documented impairment authorisation before contractor work, then separate design, pressure-test, alarm witness and formal restoration steps.

WHY THIS ORIGINAL FIRE-SAFETY CONTROL MATTERS

A detector or suppression system can be temporarily disabled while mine operations continue, exposing occupied areas to unrecognised fire risk. Even after component replacement, failed alarm cause/effect or an improperly restored pump may leave the mine without required protection. Each hold gate links actual evidence, date/time and responsible approval to the next activity.

FIVE ORIGINAL WITNESSED HOLD/RELEASE STAGES

1. Formal mine fire-system impairment authorisation. 2. Approved design/cause-effect revision and affected area isolation. 3. Pressure or functional-test safety approval.

4. Alarm, interlock and control-room witnessed testing. 5. Fire protection reinstatement and final close-out.

ORIGINAL EDITABLE WORD FIELDS AND MINE PERMISSION

The actual source table has FIVE numbered HOLD / STOP POINTS, STATUS (Hold / Release / N/A), and VERIFIER / RECORD (evidence, time and signature). Identifiers include Mine/site, Area/equipment, Supervisor, Company, Permit/work order and Host-mine work owner. Separate FINAL CONTRACTOR ACCEPTANCE and HOST-MINE RELEASE signature/date appear at the end; this is NOT the seven-row alarm test register.

NO RELEASE IF a required permit, technical/witnessed hold or mine acceptance is missing. An impairment, trial functional test, control-room witness and final owner handback are separate decisions; the contractor may not authorise restoration solely through its own signature.

INDIVIDUAL WORD OR COMPLETE MINE FIRE CONTROL FILE?

This exact source-original purchaser-licensed editable Word DOCX is ALREADY INCLUDED in the full 386-document Surface Mining Checklist Package and complete Surface Mining Contractor Health & Safety File. Buyers of the larger file/package receive this item within the collection together with many other mine management and fire-related controls; no separate Single purchase is required. The mine fire owner retains all critical impairment and reinstatement decisions.
