PURPOSE OF THE SOURCE ORIGINAL

ACTUAL PRINTED WORD CRITERIA
Eight original checks: confined atmosphere/entry/rescue, height and scaffold access, load-bearing lift points and signal crew, overhead isolation from live plant, emergency retrieval, competence and inspection, changed-condition STOP, closeout.
EVIDENCE AND ACTUAL ACCEPTANCE
For each of the eight original source controls enter YES, NO or justified N/A. Preserve the corresponding approved risk assessment, station specification, permit, inspection/test certificates, measurement and technical witness evidence. A tick or signature does not substitute for an operating approval.
HAZARDS, DEFECTS AND TECHNICAL ACCOUNTABILITY
Record the actual isolation or test plan, exposure controls, competent person, accepted evidence and open defect owner. Escalate electricity, steam pressure, fire impairment, hazardous atmospheres, environmental releases, lost workforce competency and unsafe SIMOPS. Link incident investigations and technical NCRs to their own controlled records; do not conceal unaccepted work or invent compliance.
SEPARATE STATION OPERATING AUTHORITY
The facility's electrical switching officer, plant controller, permit issuer and relevant technical owner independently control hazardous entry, isolation removal, energisation, critical tests and return to service. An AVC contractor form is not a station permission. Mark unverified conditions HOLD or stop work until an appointed authority issues the necessary real release.
