178 - AN-C02 | Lifting & Material Handling Final Quality & Operator Handover
WHERE / WHEN & WHY THIS DOCUMENT IS USED
Use this after the specialist operation is physically complete but before the operator accepts the affected equipment or process workface back. It checks technical quality, tests, temporary-barrier removal, open defects and permit closure so that a contractor finish declaration is not mistaken for permission to operate. Here the trade is lifting and material handling, with a source-identified process exposure of lift impacts live process pipework or overhead services. Link the checks to actual equipment tags and the current facility-authorised scope.
THE SOURCE-PRINTED CONTROLS
The document contains EIGHT printed PROCESS / SAFETY VERIFICATION rows, each with Y, N and N/A decision boxes: (1) Work completed against approved scope and technical specifications; (2) Process over-sail exclusion; (3) Lift exclusion and route release; (4) Post lift mechanical condition; (5) Temporary barriers, tools, materials and waste removed / accounted for; (6) All isolations and permits returned through operator-controlled process; (7) Remaining defects/NCR and operating restrictions documented and accepted; (8) Area/access handback.
HOW TO COMPLETE THE RECORD
Enter FACILITY / UNIT / EQUIPMENT, the true work order and operator PTW reference, plus contractor/shift. Inspect actual conditions, record EVIDENCE / OPEN ACTION / PERMIT VARIATION, and explain any N/A. The form requires separate CONTRACTOR / NAME & SIGN and FACILITY / AUTHORITY & SIGN with DATE / TIME. Keep underlying inspection, SDS, test or permission evidence as relevant to these exact controls.
STOP, HOLD & AUTHORITY LIMITS
The original HOLD RULE is explicit: an unanswered critical item, absent plant permit or unapproved deviation requires STOP/HOLD. A contractor tick, internal signature or completed checklist does not authorise release from an operating refinery. Resolve deficiencies with the competent technical reviewer and facility operator, and record the approved decision before progressing.
TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE
Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
