167 - AM-R02 | Welding & Fabrication Process Deviation & Closeout Register
WHERE / WHEN & WHY THIS DOCUMENT IS USED
Use this register when a refinery workpack develops an unexpected condition, technical deviation, changed site interface or unresolved quality decision. It preserves the specific plant item, hazard, evidence and dated competent disposition across shifts and until controlled closeout. Apply it to welding and fabrication; the original specialist checks particularly address weld procedure/qualified welder and material traceability verified and oxygen/fuel gas cylinders and flashback protection inspected. An undocumented change at this interface can remain dangerous after a shift change or completion declaration.
THE SOURCE-PRINTED CONTROLS
The printed four-column working record is: (1) PLANT ITEM / WORK ORDER; (2) DEVIATION / HAZARD; (3) DECISION / CONTROL / EVIDENCE; (4) RELEASE AUTHORITY / DATE. Enter the actual facility tag/work order, the unexpected condition, the technical decision and evidence, then the named authority and decision date. Preserve the supporting permit change, test result, engineering/QA disposition or nonconformity as applicable.
HOW TO COMPLETE THE RECORD
Complete facility/unit/equipment, WORK ORDER / PTW REF. and CONTRACTOR / SHIFT. Appoint OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY to keep unresolved matters visible. When a change is accepted, add actual supporting files, obtain separate CONTRACTOR and REVIEWER signatures, date/time, and retain it with the workpack.
STOP, HOLD & AUTHORITY LIMITS
Recording a deviation does not itself approve a new method, remove a HOLD or permit plant restoration. Where changes affect process boundaries, isolation, hazardous operations or test/quality acceptance, stop the affected work and obtain an operator-controlled and competent technical decision before any continuation.
TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE
Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
