166 - AM-R01 | Welding & Fabrication Competence & Inspection Evidence Register

WHERE / WHEN & WHY THIS DOCUMENT IS USED

Use this before and throughout the specialist work to link specific workers, tools and test assets to appropriate approvals, operator permits, inspection evidence and expiry dates. A generic training file cannot prove the nominated person or instrument is currently authorised for this particular workface. Apply this to welding and fabrication, where the source trade controls include weld procedure/qualified welder and material traceability verified. The register is a practical traceability tool—not a licence for someone to work.

THE SOURCE-PRINTED CONTROLS

The four original table headings are: (1) PERSON / ASSET IDENTIFIER; (2) QUALIFICATION / APPROVAL; (3) PERMIT / PTW REF; (4) TEST / EXPIRY / FILE REF. Enter the actual person, plant item or test asset; evidence of relevant qualification/approval; the real operator-issued PTW; and test/certificate reference and validity. Use actual identifiers and file references, not a tick saying only “trained”.

HOW TO COMPLETE THE RECORD

In this welding and fabrication workpack, test the record against source-specific controls such as fire watch and screen placement approved and classified area controls and nearby drains/lines protected. Retain signed permits, competence appointments, test results and asset inspections where applicable. Identify OWNER / CUSTODIAN, NEXT REVIEW / EXPIRY and get the contractor/reviewer names/signatures/date/time.

STOP, HOLD & AUTHORITY LIMITS

An entry does not create competence, authorise specialised regulated work or replace the facility's permits. If training, calibration, inspection approval or an underlying permit is absent/expired, the dependent task remains unreleased until correct evidence and site authority are obtained.

TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE

Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
