143 - AK-C03 | Industrial Cleaning Unexpected Condition STOP-WORK
WHERE / WHEN & WHY THIS DOCUMENT IS USED
Use this if the operating condition, permit boundary, hazard, scope or approved method unexpectedly changes. The original STOP-WORK form provides a documented, joint reassessment route, rather than treating an altered refinery condition as a normal site instruction. Here the trade is industrial cleaning, with a source-identified process exposure of injection injury, toxic aerosol release or uncontrolled vacuum/hose energy. Link the checks to actual equipment tags and the current facility-authorised scope.
THE SOURCE-PRINTED CONTROLS
The document contains EIGHT printed PROCESS / SAFETY VERIFICATION rows, each with Y, N and N/A decision boxes: (1) Current plant status and workface boundary reconfirmed with the facility area owner before activity; (2) Injection injury, toxic aerosol release or uncontrolled vacuum/hose energy; (3) Confirm jetting pressure and equipment rated condition, hose restraints, receiving containment, operator exposure and drain discharge routes; (4) New process, energy, chemical, ignition or SIMOPS condition recorded and work stopped pending reassessment; (5) Operator-controlled permit and isolation validity reconfirmed; no informal supervisor-only restart; (6) Affected adjacent trades, area operations and emergency response personnel advised of the deviation; (7) Engineering / environmental / quality nonconformity disposition filed where the change affects integrity; (8) Competent contractor supervisor and operator area authority jointly document permitted restart or cancellation.
HOW TO COMPLETE THE RECORD
Enter FACILITY / UNIT / EQUIPMENT, the true work order and operator PTW reference, plus contractor/shift. Inspect actual conditions, record EVIDENCE / OPEN ACTION / PERMIT VARIATION, and explain any N/A. The form requires separate CONTRACTOR / NAME & SIGN and FACILITY / AUTHORITY & SIGN with DATE / TIME. Keep underlying inspection, SDS, test or permission evidence as relevant to these exact controls.
STOP, HOLD & AUTHORITY LIMITS
The original HOLD RULE is explicit: an unanswered critical item, absent plant permit or unapproved deviation requires STOP/HOLD. A contractor tick, internal signature or completed checklist does not authorise release from an operating refinery. Resolve deficiencies with the competent technical reviewer and facility operator, and record the approved decision before progressing.
