140 - AJ-TB-02 | Working On Tank Roofs And Floating Roofs Toolbox Talk
WHERE / WHEN & WHY THIS TOOLBOX TALK IS USED
Use this practical toolbox talk before working on tank roofs and floating roofs in the tanks and tank farms workpack of an operating refinery, petrochemical facility or other process installation. The original form identifies this exposure: Tank atmosphere or pyrophoric residue ignition during entry and cleaning. Explain the actual equipment, current plant state, site work order and permit boundary; use the workface equipment to demonstrate the topic rather than simply reading a general safety lecture.
THE SOURCE-PRINTED CONTROLS
The original Word form requires the facilitator to cover these FIVE specific discussion/demonstration points: (1) Check permitted load and stability of fixed or floating roof surfaces; (2) Avoid unplanned penetration of fragile or corroded decking; (3) Use lifelines and fall rescue systems acceptable for tank conditions; (4) Coordinate vapour emissions and roof movement with tank operations; (5) Account for dropped objects and close all openings before handback. These must be related to the actual equipment, material and operating area.
HOW TO COMPLETE THE RECORD
Before starting, cross-check the source-defined escalation boundary: Verify product history, sludge/sulphide hazards, ventilation and gas-test sequence; no entry without operator confined-space authority. If the relevant exposure control, permitted method or plant condition changes, stop affected work and notify the operator permit issuer or area authority. Document reassessment and formal release before resuming; a toolbox talk is not an operating permit.
STOP, HOLD & AUTHORITY LIMITS
Fill in FACILITY / UNIT / EQUIPMENT, WORK ORDER / PTW REF. and CONTRACTOR / SHIFT. Ask the crew which hazard can change today, who owns the isolation and what proves permission to restart. Record actions or questions, facilitator name/signature/date/shift and employee IDs/signatures for up to 16 attendees. Keep the signed briefing with the relevant workpack.
TRY ONE, THE PACKAGE OR THE COMPLETE SAFETY FILE
Download this original editable A4 Word (DOCX) record individually for R12.95. The 260-document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95—only R340 more than the package—and contains the same 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist annexures. Read individual listings to understand a form before choosing a larger set. Prices may change; templates are not accredited or operator-issued permits.
