Use the original printed criteria, source register columns, toolbox attendance or HOLD gates as applicable. Record authentic work order, competent jetting/cleaning staff, approved chemical SDS, issued area and pressure controls, prestart evidence, signed QA witness, as-left cleaning and waste/chemical disposal. Do not fabricate results, discharge permission, operator handover or source signatures. A blank field is not technical acceptance.
