055 - AB-TB-05 | Permit & Isolation Shift Handover Toolbox Talk

When and why this refinery crew briefing matters

Use this contractor toolbox talk on permit and isolation shift handover before work at a real refinery or petrochemical process area. The source instruction is to brief the actual task, process condition, controlled workface boundary and stop-work criteria; it is not a classroom-only generic safety handout.

The actual source discussion and demonstrations

The original source provides these practical facilitator discussions: A shift change can break the link between a written permit and the actual plant condition. Each incoming team must verify the workface, isolation tags, unfinished operations and restrictions, not rely solely on a verbal assurance. The outgoing supervisor identifies all open permits, suspended work, gas-test conditions, plant bypasses and temporary barriers. The incoming supervisor checks those items with the permit issuer and area controller. Do not remove a lock, tag, blind, earth or barricade merely because it was placed by the previous shift. Follow the facility's formal custody-transfer and isolation-release procedure.

What to stop, check or escalate on site

Discuss the remaining actual source stop/release guidance with the crew: Explain all open defects, simultaneous operations, safety-system impairments and test holds. Use the controlled handover record and ensure workers know which tasks are not yet authorised to resume. Ask an incoming worker to identify the responsible area authority and the isolation boundary. If any record, tag or permit is missing, the affected work stays on HOLD pending revalidation.

How to complete the original attendance record

At the top fill in FACILITY / UNIT, CONTRACTOR, WORK ORDER and AREA / SHIFT. Ask attendees to point out the correct work boundary, live-plant hazards and who can approve changed conditions. The Word talk provides a numbered employee PERSONNEL ID / SIGNATURE attendance table for 16 people. File the original signed page with the task workpack; a briefing does not grant permission to work or restart.

INDIVIDUAL DOCUMENT OR COMPLETE REFINERY SAFETY SYSTEM?

Read the single to understand what the particular record does, then choose the set that fits your scope. Prices may change.
