016 - AB-13 | Pressure Equipment, Tests & Mechanical Integrity

Where this compulsory refinery interface check applies

Use AB-13 for pressure equipment, tests and mechanical integrity within an active refinery/petrochemical contractor safety-file workpack. It is a process-plant Section AB interface check, intended to make the actual operator, site owner, process unit and contractor responsibilities visible before the relevant work stage. The source form says it is completed in accordance with the site permit and work package.

Eight actual printed source checkpoints

The source has eight numbered CONTROL / EVIDENCE rows, each YES, NO or N/A: 1. Equipment type, design basis, pressure and inspection boundary agreed; 2. Operator identifies PER applicability, authorised inspection roles and certificates; 3. Testing method, medium, exclusion zones and safe test pressure approved; 4. Gauges, test hoses and instruments rated, identified and calibrated; 5. No work on live pressurised equipment without separate authorised engineering method; 6. Failed results and leaks trigger isolation and NCR records; 7. Pressure test certificate and inspection/witness releases filed; 8. Operations authorises commissioning and return to service.

How to complete the real workface evidence

Enter the FACILITY / UNIT, CONTRACTOR, WORK ORDER and AREA / SHIFT. Tick each actual check, explain any N/A, enter the action/record or operator permit reference and retain real supporting certificates, risk assessments, issue/revision, operator approvals and test evidence where applicable to these eight items. Review the completed form with the workface authority.

Stop, HOLD and operator permission

The original instruction explicitly states this is a contractor evidence record that does not replace the facility permit, operating authority or emergency procedures. An unanswered critical check, changed plant state, missing isolation or expired permit must be escalated; do not interpret the form as an authorisation to work or restart.

INDIVIDUAL DOCUMENT OR COMPLETE REFINERY SAFETY SYSTEM?

Read the single to understand what the particular record does, then choose the set that fits your scope. Prices may change.
