009 - AB-06 | Isolation, Lockout & Stored Energy Readiness
Where this compulsory refinery interface check applies
Use AB-06 for isolation, lockout and stored energy readiness within an active refinery/petrochemical contractor safety-file workpack. It is a process-plant Section AB interface check, intended to make the actual operator, site owner, process unit and contractor responsibilities visible before the relevant work stage. The source form says it is completed in accordance with the site permit and work package.
Eight actual printed source checkpoints
The source has eight numbered CONTROL / EVIDENCE rows, each YES, NO or N/A: 1. Operator-defined energy boundary includes electrical, hydraulic, pneumatic, pressure, thermal and gravity sources; 2. Process contents, toxic/flammable residues and trapped energy addressed; 3. Authorised isolator and work-group personal protection verified; 4. Disconnection, positive isolation and zero-energy test accepted under plant rules; 5. Safety instrumented system bypass/interlock override controlled by authorised owner; 6. Isolation alteration or temporary bypass cannot proceed informally; 7. Shift turnover does not silently remove personal lock protections; 8. Restoration controlled after inspection and people-accountability clearance.
How to complete the real workface evidence
Enter the FACILITY / UNIT, CONTRACTOR, WORK ORDER and AREA / SHIFT. Tick each actual check, explain any N/A, enter the action/record or operator permit reference and retain real supporting certificates, risk assessments, issue/revision, operator approvals and test evidence where applicable to these eight items. Review the completed form with the workface authority.
Stop, HOLD and operator permission
The original instruction explicitly states this is a contractor evidence record that does not replace the facility permit, operating authority or emergency procedures. An unanswered critical check, changed plant state, missing isolation or expired permit must be escalated; do not interpret the form as an authorisation to work or restart.
INDIVIDUAL DOCUMENT OR COMPLETE REFINERY SAFETY SYSTEM?

The 260-document Petrochemical Checklist Package is R1,559.95; the 500-page Petrochemical Contractor Safety File is currently R1,899.95 and includes all 260 forms, A–AA, the compulsory AB process-plant interface and AC–AV specialist annexures. Read the single to understand what the particular record does, then choose the set that fits your scope. Prices may change.
